During my last call in PAC, I encountered something I've never faced before. While examining a patient, I heard like a 'kekecohan' from the front counter, in my heart, I thought it might be a BBA (born before arrival) case. So I excused myself from the patient and said that I need to attend to another case. The baby was not born yet but the patient has been pushing since the past 2 hours and even worse, her vulva was so swollen. I had never seen anything like it! It was so weird-looking. Being careful, I thought I'd better send the patient off to the labour room myself rather than leaving it to the nurses. At least I'm in the know if anything happens.
So when we got to the labour room, the nurses asked me to do VE (vaginal examination) because they were unsure of the station of the head. So I on a glove and did a VE. It wasn't the head crowning, it was the caput! I asked the patient why she had not come earlier, she said it was because she was waiting for her brother to come. She's an Indonesian with no passport, it's a common story that you hear in the labour room. Anyway, I encouraged her to bear down. I had expected that it'll be a quick delivery but I was wrong. I had decided not to do any episiotomy even though she's a primigravida because her vulva was too swollen for an epi. I thought it's better to repair the tear and hopefully it's minimal. I gave my very best to guard her perineum. After about 5 minutes, the baby was delivered with an outstanding Apgar score.. 9 at 1 minutes of life and 10 at 5 minutes of life! I was quite worried that the baby might be in distress especially when the baby had already passed meconium.
I felt sorry for her because I cannot imagine how much pain she was in... She was sweating! I didn't agree with how the other staff were talking rudely to her asking about her passport. Although she's an illegal immigrant, I believe that everyone should be treated with respect and gently when they are in pain, right? But later I clarified with another person why the nurses talk roughly to the illegal immigrants, the reason I got was because the child born will be a Malaysian, so they get pissed thinking about it. To be honest, I think it's quite a fair reason, hehe but I an against it. I just knew that Malaysia is too lenient when it comes to citizenship. Well, I dare not comment any further especially when I'm a government servant.
Erm... Speaking of government servant, I was so thrilled when I got good deals wit the hotel price! It was the first time that I felt privileged being a government officer, hehe.
I am so exhausted right now. Since my last on call I had been staying late at the hospital until 10 pm almost everyday! Lucky I get to go home early tomorrow. Plan to go to Pavillion with Tiyah and Sofiya!!! Cannot wait!

No la, it wasn't the head yang part bony tu, it was caput, the skin. so when you tengok, it looked like the station is low, but then when you palpate properly, it was just edematous skin. it gives a false impression la.
